A Diabetes MedsCheck is a service designed to provide an in-pharmacy medicine review between pharmacists and patients to enhance quality use of Medicines and reduce number of adverse medicine events. These services aim to support self-management of medicines by evaluating patient’s knowledge about their medicines. 

Diabetes MedsCheck 

A Diabetes MedsCheck is like the MedsCheck service focusing specifically on the Patient’s experience with type 2 diabetes. Diabetes MedsCheck aims to:

  • Improve patients’ use of medicine, through educating and building their knowledge about their diabetes medication therapy
  • Educate and improve patient’s knowledge about the effectiveness of blood glucose monitoring devices 
  • Reduce risks of patients developing complications related to type 2 diabetes. 
  • Develop an effective “Management Plan” to manage your blood glucose levels. 

Who can benefit from MedsCheck & Diabetes MedsCheck?

  • Recently discharged patients from the hospital 
  • Patients taking several medicines (prescription and non-prescription medicines)
  • patients who have had changes made to their medicines 
  • patients who attend more than one doctor 
  • patients who are unsure or confused about their medicines 
  • patients who have type 2 diabetes
  • patients who use devices that assist with medication management such as blood glucose monitoring or nebuliser etc.
